Exempt from the mandatory visa requirement: citizens holding valid diplomatic passports (except persons traveling to work at Ukrainian representative offices in Poland);
crew members of air and sea vessels, aircrew members and service personnel engaged in emergency and rescue operations;
citizens of Ukraine holding residence permits in Poland issued by the relevant authorities.
Exempt from the mandatory transit visa requirement: citizens of Ukraine holding visas issued by the competent authorities of one of the states party to the Implementing Convention of 19 June 1990 to the Agreement between the Governments of the Benelux countries, Germany, France, as well as the Czech Republic, the United Kingdom and Ireland.
Multiple-entry visas may be issued to: railway transport workers while performing their official duties;
drivers and crew members of vehicles engaged in international transportation;
participants in exchanges through state authorities and local self-government bodies (regardless of their field of activity);
persons participating in regular bilateral contacts in the economic, cultural, scientific, educational, sports or other important areas of cooperation;
persons holding a document confirming the right of ownership of real estate located in Poland or the right to receive an inheritance on the territory of Poland;
persons intending to visit the graves of relatives in Poland;
persons applying for such a visa to maintain confirmed family ties and to visit places of their origin, as well as members of their families and close relatives;
pensioners, provided they present the relevant documents.
Documents required to obtain a Polish visa: a visa application form, filled out in Latin letters (on a computer, typewriter, or by hand in block letters), which contains answers to all questions, with a photograph attached (3.5 x 4.5 cm);
a foreign passport or another document entitling the holder to cross the state border, valid for at least three months after the date of the planned departure from Poland;
a photocopy of the passport pages containing personal data and the photograph.
Additional documents (depending on the visa type): a permit for permanent or temporary residence in Ukraine or registration (for foreigners); a voucher confirming a hotel booking in Poland; an invitation issued by an institution, enterprise, or religious organization in Poland; an original certificate from an educational institution confirming studies, professional training, or internship in Poland; an original invitation issued on the form of the voivodeship administration;
l a certificate of obtaining a work permit in Poland, issued by the relevant territorial voivodeship administration.
How to fill out the application form Fill out the visa application form in the Latin alphabet on a computer, typewriter, or by hand in block letters, without corrections or alterations, in accordance with the corresponding entries in the document on the basis of which the trip is to be made.
All items in the application form must be filled out.
If any item does not apply to you, put a dash.
Attach to the visa application form a photocopy of the passport page containing personal data and a photograph.
Glue a photograph measuring 3.5 x 4.5 cm to the upper left corner.
Application forms that do not meet the above requirements will not be accepted.
Filling out the application form item by item
1. The entry must exactly match that contained in the international passport (transliteration).
2. Filled in by persons who previously had a different surname. If you have not had a different surname, put a dash.
3. The entry must exactly match that contained in the international passport (transliteration).
4. Enter the date of birth, starting with the year of birth.
5. Enter your identification code, if there is none - put a dash.
6. Enter the current name of the locality and country where you were born.
7. Enter your citizenship.
8. Enter the citizenship you received at the moment of birth.
9. Put a mark in the appropriate box.
10. Put a mark in the appropriate box, according to the state of affairs at the time of filling in the application form.
11. Enter the father's name.
12. Enter the mother's name.
13. Put a mark in the appropriate box, according to the documents submitted to the consulate.
14-17. Enter information about the document on the basis of which the trip is to be made, and in item 15 enter the name and location of the institution that issued it.
18. Citizens of Ukraine who are on its territory put a dash, others fill in depending on the actual state of affairs.
19. Enter your profession.
20. Employed persons enter the name, exact address and telephone number of the employer. Students enter the name and address of the educational institution.
21. Fill in in case of transit travel, otherwise put a dash.
22-24. Put a mark in the appropriate box.
25. Indicate the number of days to be covered by the validity of the visa.
26. Fill in if available, if not - put a dash.
27. Fill in in case of transit travel, otherwise put a dash.
28. Enter the names of countries that you have visited during the last five years. If there have been no trips, put a dash.
29. Put a mark in the appropriate box.
30. Enter the date on which the planned entry is to take place (the date of the first entry if you are applying for a multiple-entry visa).
31. Enter the date of the planned departure (the last one - in case of a multiple-entry visa).
32. Mark the border checkpoint through which you will enter Poland (the first one - in case of a multiple-entry visa).
33. Enter the name of the vehicle with which you will enter Poland.
34. Enter the details of the receiving party.
35-36. Put a mark in the appropriate box and detail the information below.
37-42. Should be filled in if family members included in your passport plan to travel with you. If they are included but do not intend to travel, put a dash.
43. Filled in by a foreigner applying for a visa to the commander of the border checkpoint. In other cases, a dash should be put.
45. Enter your home address in your country of residence.
46. Enter the contact phone number.
47. Enter the date and locality where you filled in the application form.
48. Put a signature.
