I. Write all your data required for filling out the application.
1. Full name (as in the foreign passport).
2. Date of birth.
3. Place of birth (country, locality).
4. Postal address with zip code to which the notification will be sent in case of winning.
5. Country of current residence.
6. Phone number and email address (if any).
7. Highest level of education achieved to date. You must indicate one of the following options: (1) only primary school, (2) secondary school, no diploma, (3) secondary school with diploma, (4) vocational school, (5) some university education, (6) completed university education, (7) some master's studies, (8) master's degree, (9) some postgraduate/doctoral studies, (10) candidate/doctor of sciences degree.
8. Country of origin of the applicant, if different from country of birth.
9. Marital status.
10. Number of children under 21 years old who are not married.
11. Information about spouse (full name, date, place and country of birth).
12. Information about children (full name, date, place and country of birth). Please note that children who are married or have already turned 21 are not listed in the application.
II. Attach to the letter a color photo of the main applicant and photos of all family members listed in the application (the photos must indicate full name). The photos must meet the requirements specified in the instructions: look directly at the camera; the head should not be tilted to the side or turned up or down and should occupy approximately 50% of the photo area; the photos must be taken against a light neutral background (like for documents, about 6x6 cm).
III. Send the letter with data and photos to the editorial address: 01001, Kiev-1, P.O. Box 386-V, 'Zagranitsa' (marked 'DV-2010') by November 25. If you have digital photos, it is better to send the application by email: [email protected]
IV. The cost of services (at the rate of 150 UAH for each individual application or 200 UAH for two applications from spouses) must be paid only after we confirm that all data is provided correctly and your application is ready for registration.

Applications will be processed and registered on the official website of the DV-2010 visa lottery. All participants will be informed about the registration - a printout of confirmation with a control number, by which you can find out the results of your participation in the DV-2010 visa lottery from July 1, 2009, will be sent to the addresses specified in the applications.
