I. Legibly (in block letters) write all your data necessary for filling out the application.
1. Full name (if you have a foreign passport, as in the passport).
2. Date of birth (day, month, year).
3. Place of birth (country, city/locality, region).
4. Postal address with zip code, to which notification will be sent in case of winning.
5. Country of current residence.
6. Phone number and email address (if available).
7. Highest level of education achieved to date. You must indicate one of the following options: (1) only elementary school, (2) secondary school without diploma, (3) secondary school with diploma, (4) vocational school, (5) incomplete higher education, (6) completed higher education, (7) incomplete master's program, (8) master's degree, (9) incomplete postgraduate/doctoral program, (10) candidate/doctor of sciences degree.
8. Country of which the applicant is a native, if different from country of birth.
9. Marital status.
10. Number of children under 21 years of age who are not married.
11. Information about spouse (full name, date, place and country of birth).
12. Information about children (full name, date, place and country of birth). Please note that children who are married or have reached the age of 21 are not listed in the application.
II. Attach to the letter passport-sized photos of the main applicant and photos of all family members listed in the application.
On the back of the photos, write the full name. The photos must meet the requirements specified in the instructions.
III. Pay for the services at the rate of 100 UAH for each individual application (150 UAH for two applications from spouses).

IV. Send the letter with data, photos, and the original or copy of the payment receipt to the editorial office: 01001, Kyiv-1, PO Box 386-V, 'Zagranitsa' (marked 'DV-2009') by November 25, 2007.
The editorial office will process the applications and register them on the official website of the DV-2009 visa lottery. All participants of the promotion will be informed about the registration - a printout of confirmation will be sent to the addresses provided in the applications.
