The new form (version 02-2003) is posted on the U.S. Embassy website http://usembassy.kiev.ua/visa_download_ukr.html. Applicants can obtain the forms from the security guard of the Consular Section of the U.S. Embassy. The form will also be sent by regular mail to each person who has scheduled a visa interview, along with a notice of the date and time of the appointed interview.
Along with the new visa application form, new requirements for the photos on the form come into effect. The photo must be unframed, fully depict the face, and be taken within the last 6 months. The applicant must look directly into the lens. The applicant must not look down or sideways; the face must occupy 50% of the entire photo. It is desirable to leave the ears uncovered. The photo size must be 50x50 mm, and the head must be in the center of the photo. The size of the head in the photo (from the top of the head to the bottom of the chin) must be 25-35 mm, and the eyes should be at a height of 28-35 mm from the bottom of the photo. Photos may be color or black-and-white, taken against a light background and printed without frames.
Please note: as of April 1, 2003, old visa application forms (version 08-2001) and non-standard photos will not be accepted.
