In the case of family reunification with a citizen of the Czech Republic, a certified copy of the marriage certificate (for spouses), a certified copy of the birth certificate (for an unmarried child under 18 years of age), another document confirming the family relationship (for elderly parents) shall be submitted; in the case of family reunification with a foreigner, a certified copy of the marriage certificate (for spouses), a certified copy of the birth certificate (for an unmarried child under 18 years of age), another document confirming the family relationship (for elderly parents) shall be submitted.
A personal affidavit of a family member from the Czech Republic on the obligation to provide (wife, daughter, son, single mother or father) with financial and material means for the entire period of stay in the Czech Republic. The guarantor's signature must be notarized. The said affidavit shall be accompanied by a bank statement of the account. The account must hold an amount equal to 15 subsistence minimums for personal needs for the first month plus double the subsistence minimum for each subsequent month during one year (total 86.000 CZK).
For children under 18, the financial amount is 50%. A confirmation of the guarantor's monthly salary may also be submitted.
A document on housing provision (landlord's certification) specifying the period, with the landlord's certified signature and consent to provide housing to the foreigner for the period of his/her stay in the Czech Republic. Such a document includes, in particular:
a statement by a natural person who is the owner of the property about providing housing to the foreigner, certified by signature and his/her consent to the foreigner's accommodation for the period of his/her stay on the territory of the Czech Republic;
a statement by a legal entity that is the owner of the property about providing housing to the foreigner. If the accommodation is in rented property, a certified copy of the lease agreement with the property owner must be submitted;
in the case of renting an apartment, submit a certified copy of the lease agreement with the written consent of the property owner to rent the apartment;
in the case of renting an apartment or part of an apartment, submit a certified copy of the rental agreement and the written consent of the owner or owners of the property;
in the case of accommodation in a dormitory or in a guesthouse or other establishment providing accommodation services, the foreigner submits the original confirmation (with the director's signature and the company's seal) about providing his/her accommodation, specifying the period.
An extract from the "Criminal Register of the Czech Republic" and a similar document from the state of which he/she is a citizen, or in which the foreigner is staying or has stayed continuously for the last 3 years for a period of more than 6 months.
The requirement to submit an extract from the "Criminal Register of the Czech Republic" is considered fulfilled when the foreigner submits a request for this extract to the embassy and consents to its sending directly to the competent authority - SСРР.
A foreigner under 15 years of age does not submit a criminal record document.
A copy of the applicant's passport page with his/her data and a copy of the guarantor's passport with confirmation of a permit for long-term or permanent residence in the Czech Republic.
An application for a visa for more than 90 days for the purpose of family reunification may also be submitted to the services of the directorate of the border police and the alien police of the police presidium of the Czech Republic in the following cases:

The visa is granted to a minor or an adult non-self-sufficient foreigner for the purpose of cohabitation with a legal representative (family member) who is staying on the territory of the Czech Republic on the basis of a permit for permanent residence or for a stay of more than 90 days, or is a citizen of the Czech Republic.
