For an application for a visa for a stay in the Czech Republic exceeding 90 days, a foreigner must submit: a travel passport, a completed visa application form with a photo.

A document confirming the purpose of stay in the Czech Republic. For the purpose of employment, such a document is a certified copy of the decision of the Labour Bureau for an individual foreigner. This decision cannot be used for recruiting other employees, as it is valid only for one person; a document on accommodation. Such a document is considered, in particular:
a handwritten certification by a natural or legal person (company, organization about providing accommodation to a foreigner, on which the signature of the owner of the property is notarized;
confirmation of the provision of accommodation by a facility that officially provides accommodation services (dormitory, rooms for single people, hotel, motel, etc.), signed by the responsible manager of the facility and confirmed with the company seal;
in the case of renting an apartment or real estate by a natural or legal person, it is necessary to present the original lease agreement or its certified copy;
in the case of renting an apartment by a natural person, it is necessary to present a certified copy of the rental agreement, as well as the written consent of the owner of the property, i.e., the landlord, or a certified copy;
Extract from the "Register of Punishments of the Czech Republic" and a similar document from the state of which he is a citizen, or in which the foreigner is staying or has stayed continuously for more than 6 months. The requirement to submit an extract from the "Register of Punishments of the Czech Republic" is considered fulfilled when the foreigner submits a request to the embassy for obtaining this extract and gives consent for it to be sent directly to the competent body of the directorate of the border police and the alien police service of the police presidium of the Czech Republic.
