For an application for a visa for stay in the Czech Republic for more than 90 days, a foreigner must submit: a travel passport (returned back), a completed visa application form with a photograph.

A document confirming the purpose of stay in the Czech Republic; in particular, this document is:
for the purpose of performing work in one's profession/entrepreneurship:
a certified copy of the Živnostenský list (trade license) and a certified copy of the entry in the Commercial Register of a natural person, or
a confirmed letter to the relevant Živnostenský office notifying the intention to engage in entrepreneurship, or
a letter from the Živnostenský department of the local/municipal authority requesting the removal of an obstacle - in granting a long-term residence permit in the Czech Republic, or
a certificate from the local municipality about registration in the registry of self-employed farmers;
for the purpose of entrepreneurship/participation in a legal entity (established in accordance with the Commercial Code), a member of a trading company (V.O.S.), a member of a limited partnership, a member of the board of directors, a member of the supervisory board, a secretary, a member of a cooperative and the like must provide a certified copy of the founding agreement or a notarial record of the establishment or amendment of the trading company or cooperative in which the foreigner will participate as a participant or a member of a limited partnership, or as a member of the board of directors, or member of the supervisory board, or as a secretary, or member of the cooperative. If the legal entity was established in accordance with the Commercial Code and is already included in the Commercial Register, and the foreigner subsequently participated in it, a certified copy of an extract from the Commercial Register confirming the existence of this legal entity and a document proving that the foreigner is a participant in this legal entity must also be submitted.
A document on securing accommodation with the period specified. In particular, the following are considered such documents:
a statement by a natural person who is the owner of the real estate about providing accommodation to the foreigner, certified by the landlord's signature and consent to provide accommodation to the foreigner for the period of his stay in the Czech Republic;
a statement by a legal entity that is the owner of the real estate about securing accommodation for the foreigner. If accommodation is to be provided in rented real estate, a certified copy of the lease agreement with the owner of the real estate must be submitted;
in the case of renting an apartment, submit a certified copy of the lease agreement with the written consent of the owner of the real estate to renting the apartment;
in the case of hiring an apartment or part of an apartment, submit a certified copy of the tenancy agreement and the written consent of the owner or owners of the real estate;
in the case of accommodation being provided in a dormitory or boarding house or other facility that provides accommodation services, the foreigner submits the original confirmation (with the director's signature and the company's seal) about securing accommodation with the period specified.
Financial means for stay in the Czech Republic:
for the purpose of entrepreneurship as a natural person holding a Živnostenský list, in the amount of 50 subsistence minimums established by law (116.000 CZK);
for the purpose of entrepreneurship as participation in a legal entity, in the amount of 15 subsistence minimums, i.e., 34.800 CZK ($1.050), and for each subsequent month this amount increases by double the amount of the subsistence minimum, i.e., 4.640 CZK ($140), in total 86.000 CZK.
The financial means must be in Czech crowns in a personal bank account, or in the equivalent in US dollars in a bank account of a bank in the Czech Republic or a foreign bank. In the said case, the availability of funds must be confirmed by an international credit card and a confirmation of the account balance (certified translation into Czech).
An extract from the Criminal Register of the Czech Republic and a similar document from the state of which he is a citizen, or in which the foreigner has continuously stayed or has stayed over the last 3 years for a period of more than 6 months. The requirement to submit an extract from the Criminal Register of the Czech Republic is considered fulfilled when the foreigner files an application for this extract with the embassy and consents to its being sent directly to the competent authority - SCPP. A foreigner under 15 years of age does not submit a document on criminal record.
